MAKE PAYMENTS

FORMS

There are many forms that we need to collect to make sure we have the most up to date information for every student in the band. In the event of emergencies, or just incase we need to contact home. All documents to be turned into Mr. Elias.

DOCUMENTS

There are a few documents that we like to make sure that every student has. Whether they are directly related to music or not. Documents regarding health, upcoming events/concerts/clinicians will be made available.

CONTRACTS

Being a valued member of this program requires a certain level of commitment from everyone involved. Our contracts are to outline clear expectations and requirements and how those things tie into your participation and in-class academic performance.

PHYSICAL FORM

FEILD TRIP AUTHORIZATION FORM

Please download and fill out clearly and correctly. This form is to provide consent for your student to travel with the program. Some events will require additional clearance from parent(s) and/or guardian(s).

INSTRUMENT RENTAL CONTRACT

This contract is for any students looking to rent an instrument from the school. There is a fee attached to every rental. 

School Year: $20

Summer: $10

This document is to be used any students trying to get into a band class.
